Dosage Request Form

Please complete this form to request a dosage calculation for your specific project.  In all cases, we need the project size information such as square footage and concrete thickness.

If you have an existing rebar or mesh design, simply provide that information and we can generate a design based on that.  However, if you do not have an existing rebar or mesh design, please complete as much of the load, soil, etc. information as possible.  The more information you can provide the better.

There are four sections available on this form should your project have more than one reinforcement configuration or multiple types of applications (such as slab on grade, paving, foundation, etc.)

Make sure you also complete the contact information portion.  When you have completed the form, you can return it via email to engineering.support@helixsteel.com or via fax to 734-786-1633.

Instruction notes for downloading and submitting the Helix Dosage Request Form.
1.  Click on the link below.  This will open the form in your web browser.
2.  You will then either need to save the file or select "Open in a Different Viewer".  This will allow you to view the file in Adobe Acrobat/ Reader where you will be able to complete the form and submit.
3.  Once you have opened the file in Adobe, fill out the form as completly as possible.
4.  Once the form is complete, click "Submit Form" in the upper right corner to save the form to submit via email.   If you would prefer, you may also print the form at this time and return it via fax.
5.  Follow the prompts to save and email the completed Helix Dosage Request Form.
